Three amounts can mean different things
A list price is a published drug-price benchmark. The insurer’s allowed amount is the basis used under the plan’s arrangement with the provider. Your personal cost share depends on coverage and plan rules, including remaining deductible, coinsurance, network and any assistance that actually applies. One amount cannot substitute for another.
The policies discussed below address TED infusions under medical benefits. Ask which benefit applies to your own plan rather than assuming ordinary retail-pharmacy or Part D rules. The drug, administration or facility, specialist visits, and monitoring may produce separate charges. Travel and time away from work add practical costs.

Ask these questions before scheduling
- Identify the exact plan/product, state rules, benefit, policy name, version and effective date.
- Check both the prescribing clinician and the infusion site; they may be separate billing entities.
- Ask about referral, prior authorization, preferred treatment, site-of-care rules and specialty-pharmacy sourcing.
- Ask the clinic who submits authorization and the supporting medical records.
- Request separate estimates for medicine, administration/facility, visits and monitoring across the planned course.
- Ask how deductible, coinsurance and out-of-pocket limits apply, and what could change the estimate.
An authorization is not a blanket guarantee of payment. Record the representative, reference number, date and promised follow-up. Do not enter member identifiers or clinical records into this educational website.
Dated examples show why your exact policy matters
UnitedHealthcare Community Plan — medical benefit policy CS2026D0089R, effective October 1, 2026; checked October 22. It includes Lumvoa and Tepezza with condition-specific criteria for moderate-to-severe active or chronic disease plus further requirements. This is a partial education summary, not all eligibility criteria and not a statement about UnitedHealthcare commercial coverage.
The general Community Plan document does not apply universally across states. Arizona and Florida have state Medicaid paths; Ohio and Pennsylvania have separate policies; Texas points to the Medicaid manual. Kansas and North Carolina are also listed as states to which this policy does not apply. In Texas, do not automatically apply the general Community Plan policy. Ask for the exact state/product document.
Aetna commercial medical plans — Clinical Policy Bulletin 0419, last reviewed April 23, 2026; checked October 22. The bulletin addresses Tepezza and other Graves’ ophthalmopathy treatments. Applicable plan designs require precertification and site-of-care utilization review; prescribing or consultation requirements and course restrictions apply. Medicare criteria are separate. This is not a verified Lumvoa coverage conclusion. Failure to mention Lumvoa does not itself establish a denial.
Neither example is your coverage contract. Obtain the policy that applies to the actual plan and proposed service date, then ask the insurer and clinic to explain it together if the answers conflict.

Assistance depends on the program and insurance category
Amgen’s Tepezza support and Viridian’s Lumvoa/ViridianCares resources are manufacturer-sponsored. They can describe benefits support and eligible assistance, but current enrollment, expense limits and insurance conditions need direct confirmation. Do not assume financial help covers every charge.
Commercial copay programs may exclude government insurance. Medicare, Medicaid and Medicare Advantage patients should ask about options for their own category rather than assuming a commercial program applies. Ask what assistance covers, what it does not cover, and whether any funding is actually available. No dollar saving or acceptance is promised here.

If there is a denial, start with the actual letter
Get the written reason and the applicable policy. Ask the clinic and insurer whether the problem is missing records, authorization, a plan criterion, the site of care, or another issue. Write down the appeal route and deadline from the letter; deadlines are not universal.
Ask the treating team about urgent handling when medically appropriate. Do not delay urgent assessment of changing vision while waiting for insurance work. Use the worksheet below to identify the owner of the next call and the date you expect an answer.
